They were peasants who grew up poor, so her life choices in getting pregnant likely didn't scupper any plans for them. Rowena was very attached to a young boy named Oskar, someone who reminded her of the son she'd abandoned and who she grew to love, healing him of his illness and granting him immortality, not able to see him die (and grateful to his family). After Rowena left Poland she never saw him again until Crowley brought him to her: to kill. This was heartbreaking to her even though Rowena did it, something she hasn't gotten over.

They met after the Winchesters tracked her down (to kill her) over her actions, with Sam making the first move towards friendship with her: asking for her help. His calm manner and ease of conversation with her helped their banter grow into friendship, especially after he understood her trauma.
